Transcripts
Transcript requests should be submitted at least two weeks prior to your college or scholarship deadline to ensure enough time to process your request and account for mailing time.  

Where do I obtain a transcript request form?
Pick up in the Main Office, CCRC, or downloaded online (link below)


*Student signature is required*
*Parent/guardian signature is required for ALL students under the age of 18*
*STUDENTS are responsible for ensuring the address that transcripts will be sent to is correct*

COST
            -No charge for the first two (2) transcript requests for current 9th - 12th grade students    
            -$1.00 after the 2nd request for current 9th - 12th grade students
            -$5.00 after the student graduates or withdraws from WHIS


PROCESSING TIME
Within 3 business days upon receipt of original signed transcript request form and payment.

Sent via US Postal Service:  Processing time does NOT include mail transit time once it is mailed from the Registrar’s Office. 

Waialua High and Intermediate School is NOT responsible for delayed or lost mail.


OTHER IMPORTANT INFORMATION
Complete the form as CORRECTLY and COMPLETELY as possible.  CHECK your form before turning it in.

Official transcripts will only be sent DIRECTLY to the school or organization

Which counselor’s signature do I need to obtain on transcript request form (if applicable)?

Whomever you ask to write a letter of recommendation for you will be the counselor to sign your transcript request form (if needed).  The counselor signing the form is to notify the counselor that they should be expecting to receive an official or unofficial transcript from the registrar’s office to either upload online or to place into a packet that you will be mailing to the college, scholarship committee, etc.

IF…

Grade level counselor is writing your letter of recommendation = ask for GL counselor’s signature on transcript request form (if needed)

CCRC counselor or another adult/teacher is writing your letter of recommendation = obtain CCRC counselor’s signature on transcript request form (if needed)

Procedure


College Admissions Office, scholarship committee, or any other organization

 

 -OFFICIAL transcript

 

-“Mail to address below”

Complete address box on form with address where transcript needs to be sent

-Turn in transcript request to registrar’s office

 

-Registrar’s office will mail transcript

 

 

 

 

 

ONE, completed packet of documents (not multiple envelopes) to be mailed to college, scholarship, or any other organization

 

-OFFICIAL transcript

 

-“Give to WHIS Counselor (Hardcopy

ONLY)”

 

-Obtain signature from CCRC 

 

 

-Turn in transcript request to registrar’s office

 

-Gather all OTHER documents needed for your packet

 

-Once you have all other documents and your packet is ready to mail, go to the CCRC

 

-CCRC will place official transcript & any other remaining documents needed from CCRC into the completed packet

 

-CCRC will seal the now completed packet

 

-STUDENT and CCRC will take packet together to the main office to be weighed for postage fee

 

-STUDENT will pay postage fee to MAIN OFFICE

 

-MAIN OFFICE will mail packet
