Transcripts
Transcript requests should be submitted at least two weeks prior to your college or scholarship deadline to ensure enough time to process your request and account for mailing time.  

Where do I obtain a transcript request form?
Pick up in the Main Office, CCRC, or downloaded online (link below)


*Student signature is required*
*Parent/guardian signature is required for ALL students under the age of 18*
*STUDENTS are responsible for ensuring the address that transcripts will be sent to is correct*


DROP OFF (Registrar's Office in Main Office) or MAIL your transcript request AND payment (cash, money order or cashier’s check - written to Waialua High and Intermediate School) to:

COST
            -No charge for the first two (2) transcript requests for current 9th - 12th grade students    
            -$1.00 after the 2nd request for current 9th - 12th grade students
            -$5.00 after the student graduates or withdraws from WHIS


PROCESSING TIME
Within 3 business days upon receipt of original signed transcript request form and payment.

Sent via US Postal Service:  Processing time does NOT include mail transit time once it is mailed from the Registrar’s Office. 

Waialua High and Intermediate School is NOT responsible for delayed or lost mail.


OTHER INFORMATION
Complete the form as CORRECTLY and COMPLETELY as possible.  CHECK your form before turning it in.

Official transcripts will only be sent DIRECTLY to the school or organization

**If a student must submit/mail an official transcript in ONE, COMPLETE PACKET to a college or scholarship (i.e. college or scholarship requires that transcript is sent in a packet with other documents and not individually), the student must:
    1.  Complete a transcript request form
    2.  Bring the ENTIRE, COMPLETED PACKET ready to mail to the registrar's office in the administration
            office WITH the transcript request form
    3.  The registrar's office will weigh the packet and the student will be responsible for paying the postage rate
            AND any applicable transcript fees to the registrar's office
    4.  The registrar's office will enclose the student's official transcript in the packet, seal, and mail the packet for
            the student
