****Mandatory Parent Meeting****
ONE PARENT MEETING FOR ALL WAIALUA SPORTS!!!!!
If your Student-athlete is thinking or will be participating in any FALL, WINTER, or SPRING SPORTS there will be a ONE-TIME mandatory parent meeting August 6th 6:30pm in the GYM. Dinner will be provided.
SY 2025-26 Waialua Athletics
All athletic forms below must be completed before participation.

Required Documents/Procedures to participate in Waialua 2024-25 Athletics:
1) Pre-participation physical (Valid= Dated within ONE calendar year; must be printed; please turn these into the school office mailboxes of Carrie Yoshimura or Kaua Segawa, OR bring it to the next practice.)
2) Concussion management study form (must be submitted w/ pre-participation physical (ONE for the school year); must be printed; please turn these into the school office mailboxes of Carrie Yoshimura or Kaua Segawa, OR bring it to the next practice.)
3) Assumption of Risk/Consent form (a.k.a. emergency card; Valid for one season of the sport(s) listed on form; if contact information needs to be updated, then please submit a new form; must be printed; please turn these into the school office mailboxes of Carrie Yoshimura or Kaua Segawa, OR bring it to the next practice.)
4) Concussion Video & Risk Management Video (Parent & Student Athlete(s) must watch these informational videos; After watching these videos acknowledgment of parent/student athlete watching these videos should be made by marking the appropriate boxes.)
